【摘要】 目的了解新兵常见心电图的改变、意义及如何判定体检结果。方法对复检新兵进行常规12导联心电图检查,并对心电图结果按《征兵心电图诊断标准》进行分析。结果558例心电图正常,561例大致正常心电图,51例异常心电图。结论新兵心电图复检时间紧,任务重,情况复杂,对复检结论质量要求高。因此心电图检查必须要结合临床、病史、心脏听诊、血压、心脏彩超等综合判断,必要时复查心电图,追踪观察心电图以做出正确合理的诊断。

心电图检查是发现心脏疾患的一个最常用的无创性检查,健康的心脏是保证新兵适应部队生活的需要,心电图是新兵体检的必须项目,笔者对2011年冬季入伍的某部1200名新兵进行了常规心电图复检工作,旨在研究新兵常见的心电图改变及意义探讨。
1对象与方法
对象2011年入伍的某部1200名新兵,其中男1080名,女120名;年龄17~25岁,平均21岁。
方法体检者在安静状态下平卧,采用深圳理邦公司生产的SE-12数字式十二通道心电图机描记。
2结果
按2009年国务院、中央军委、卫生部制定的'征兵体检心电图诊断标准并结合黄宛《临床心电图学》(第6版)分析心电图,正常心电图588例,大致正常心电图561例(见表1),异常心电图51例(见表2),对异常心电图进行心脏听诊,复查心电图、心脏彩超等相关检查,其中室性期前收缩消失3例,非阵发性房性心动过速消失1例,房室阻滞消失1例。显著窦缓嘱病人运动后心率增快达90次/min或超过原有心率30次/min,心律不齐吸气后屏气,心律不齐减轻或消失,视为正常。表1561例大致正常心电图的构成比及发生率表251例异常心电图构成比及发生率 3讨论
窦性心律失常较常见,窦性心动过速是人体生理或病理的应激反应的表现,大多数体检时发生的窦速是因为交感神经张力增高所致,查体者因空腹、紧张导致窦速,非是病理性的,不需要治疗。窦性心动过缓常见于健康成人,尤其是运动员、成人、老年人,只有当心率过于缓慢并有临床症状时,才应针对病因进行治疗。窦性心律不齐常与呼吸时迷走神经张力变化有关,屏住呼吸后可改善或消失。
缩短心电图特征表现为P-R缩短,QRS波群的宽度正常,无预激波。近来的研究认为交感神经张力增高和房室结解剖结构的缩短是可能的原因,如伴发室上性心动过速则称LGL综合征,越来越多的证据表明,单纯只有短PR间期和正常QRS间期,而没有心动过速的患者很可能是人体的正常变异[1]。
左心室高电压多见于高血压患者,其次是体型较瘦的年轻人,由于皮下脂肪较薄,电极距心脏的距离较近,尤以V5、V6导联QRS 波群振幅增高多见。排除左心室高电压体检者,可结合测血压、心脏听诊协同判断,如无异常,视为大致正常心电图。
早期复极综合征在心率较慢时以R波为主的导联J点抬高,ST段呈凹面向上抬高小于,伴T波高耸,早期复极综合征为较常见的正常心电图变异,李澜等[2]报道健康人群中检出率为,对于无自觉症状、无心脏病史的年轻男性,多考虑为良性心电图改变。
呈rSr’型或rsr’型又称室上嵴图形,r>r’ r’<,低一肋r’消失,室上嵴的除极正常时,即是心室除极QRS的终末部被掩盖,室上嵴除极相对延迟时而显露,是心电图QRS的一种正常变异。
期前收缩是异位激动点自律性增高或折返激动所形成,动态心电图广泛用于临床证明了几乎90%左右的正常各年龄组检查者均可见偶发期前收缩,检出率随年龄增高而递增,特别是正常健康人出现偶发期前收缩在临床意义上不大,频发期前收缩需要结合临床。
改变对心肌缺血的诊断并不具有特异性,必须结合临床表现及其他辅助检查,进行综合判断才能做出正确的判断。基层卫生单位新兵心电图复检具有时间紧,任务重,对复检结论质量要求高等特点,《关于调整部分征兵体检标准问题的通知》中规定:正常或大致正常心电图合格,心电图出现异常改变不一定反应有器质性心脏病,新兵常因环境变换、情绪改变、疲劳、吸烟、检查时憋气等原因而导致心电图出现一过性“边缘性”异常改变,导致“假阳性”结果。如果仅凭一份心电图就对部分新兵做出不合格的结论,势必不合格范围扩大,造成误判。另外,少数新兵明知自身存在心电图异常如心律失常等,但由于参军愿望强烈,可能存在有意隐瞒病史,并通过服用一定药物来改善或控制心电图异常表现,从而导致心电图检查出现“假阴性”结果,增加了心电图的判断难度。所以心电图检查要结合临床、病史、心脏听诊、血压、心脏彩超等综合判断,必要时复查心电图,追踪观察心电图以做出正确合理的诊断。
