SCI收录全世界出版的数、理、化、农、林、医、生命科学、天文、地理、环境、材料、工程技术等自然科学各学科的核心期刊约3500种。

作为一部检索工具,SCI一反其它检索工具通过主题或分类途径检索文献的常规做法,而设置了独特的“引文索引”(Citation Index)。即通过先期的文献被当前文献的引用,来说明文献之间的相关性及先前文献对当前文献的影响力。

SCI以上做法上的特点,使得SCI不仅作为一部文献检索工具使用,而且成为科研评价的一种依据。科研机构被SCI收录的论文总量,反映整个机构的科研、尤其是基础研究的水平;个人的论文被SCI收录的数量及被引用次数,反映他的研究能力与学术水平。

Most of us have struggled at some point with sleep. Whether it’s not getting enough sleepor struggling to get up in the morning, it can be difficult to get the balance just right.

1. The snooze button

Don’t EVER hit the snoozebutton. It really is much more beneficial to just get up on yourfirst alarm. Think about it – the snooze button gives you an extra 10 minutes or so sleep. Inthe grand scheme of your day this really won’t provide you with any more energy. In-fact itdoes the opposite. Research has shown that ‘interrupted sleep’ can cause us to feel moretired.

This means going to bed at roughly the same time each night and getting upat roughly the same time each morning. If you’re disorganized with your sleeping routine, youend up interrupting your natural sleeping rhythms, which can cause insomnia and fatigue.

The reality is that it may initially induce sleep, however it usually drasticallyimpairs sleep during the second half of the night which leads to interrupted sleep patterns thatwill leave you feeling fatigued in the morning (not to mention the hangover!)

睡前玩iPad会影响睡眠质量

Using tablet computers like Apple’s iPad and Samsung’s Galaxy Note just before bed can lead toa poor night’s sleep, according to research.

More and more people are taking their tablets to bed with them to surf the web, checkFacebook or email before switching off the light.

But researchers are warning that the blueish light their screens emit can stop users getting agood night’s sleep.

That is because this type of light mimics daylight, convincing the brain that it is still daytime.

Blue light suppresses production of a brain chemical called melatonin, which helps us fall is because our brains have evolved to be wakeful during daylight hours.

By contrast, light which is more orange or red in tone does not suppress melatoninproduction, perhaps because our brains recognize it as a cue that the day is ending.

Neurologists have known for years that staring at screens late in the evening can disrupt sleep- be they television screens, computer screens or mobile phone screens.

However, because mobiles and tablets are by nature portable - not to say addictive - morepeople are taking them into the bedroom.

Users also tend to hold them much closer to their eyes than a computer or television screen.

Researchers at the Lighting Research Centre, at the Rensselaer Polytechnic Institute in NewYork, are warning that looking at tablet displays for more than two hours “leads to asuppression of our natural melatonin levels as the devices emit optical radiation at shortwavelengths” - in other words, they emit bluer light.

They say: “Although turning off devices at night is the ultimate solution, it is recommendedthat if these devices are used at night displays are dimmed as much as possible and that thetime spent on them before bed should be limited.”

They drew their conclusions after measuring melatonin levels in 13 volunteers, after they hadspent time viewing iPads at full brightness at a distance of 10 inches, for two hours.

Melatonin levels were significantly lower after they had done this, than they were after thevolunteers had viewed their iPads for the same time, but while wearing orange glass goggles,which cut out the blue light.

They wrote in the journal Applied Ergonomics that tablet makers could "tune the spectral powerdistribution of self-luminous devices" so that they disrupted the sleep patterns of users less.

It is not just a good night’s sleep that could be jeopardized by too much late night screentime.

Researchers know that persistent disruption to sleep patterns can lead to an increased risk ofobesity, and even breast cancer.

However, these studies tend to be comparisons of those with chronic sleep disruption, suchas long term shift workers, with those who have normal sleep patterns.

羟乙基淀粉的药代动力学较为复杂,与分子量和摩尔取代度密切相关。当静脉给予本品时,低于肾阈(60,000-70,000道尔顿)的小分子很容易通过肾脏经尿排泄,大分子羟乙基淀粉在通过肾脏排泄之前,被血浆α淀粉酶降解为小分子。本品在输入体内后,血浆中羟乙基淀粉的平均分子量为70,000-80,000道尔顿,在治疗期间保持在肾阈值之上。本品分布容积约为升,输注本品30分钟后,血药浓度为最大血液浓度的75%,6小时后降至14%。单次给予羟乙基淀粉500ml,血药浓度在24小时后几乎回到基线水平。单次给予本品500ml后,药物的血浆清除率为为×h,t1/2α为小时,t1/2β为小时,药物的体内药代动力学显示非线性特征。对轻度至重度肾功能不全者进行本品的药代动力学研究,受试者单次给予本品500ml,结果显示,药物的AUC有中等程度的增加,药物在肌肝清除率ClCr<50ml/min的受试者体内AUC为ClCr≥50ml/min受试者体内的倍(95%可信限为-)。肾功能不全不影响药物的消除半衰期和Cmax。当ClCr≥30ml/min时,59%的药物经尿排泄;当ClCr<30ml/min时,51%的药物经尿排泄。对受试者进行的研究显示,每日给予10%羟乙基淀粉130/溶液500ml,连续给药10天,药物在血浆中没有出现明显的蓄积现象。在大鼠模型试验中,每日给予本品按体重,连续给药18天,在末次给药后第52天对组织的药物含量进行检测,结果显示,仅有给药剂量的在组织中储存。尚没有透析疗法对本品药代动力学影响的研究。羟乙基淀粉:是耶?非耶?作者:李妍 不能为脓毒症患者带来益处丹麦和瑞士学者检索循证医学图书馆、MEDLINE等数据库,选取2012年9月前发表的比较HES 130/ 和晶体液或人白蛋白对脓毒症患者液体复苏效果和安全性的随机对照临床试验(RCT),最终纳入9 项研究(涉及3456例患者)进行分析。结果显示,与晶体液或白蛋白相比,使用HES 130/ 对脓毒症患者进行液体复苏增加肾脏替代治疗[相对危险度(RR)为]、急性肾功能衰竭(RR=)和红细胞输注风险(RR=),且出现严重不良事件患者更多(RR=)。论文2月15日发表于《英国医学杂志》(BMJ)。HES增重症患者急性肾衰和死亡风险加拿大学者检索MEDLINE、EMBASE等数据库,选取2012年10月前发表的比较在重症患者中使用HES和其他复苏液(包括晶体液、白蛋白和凝胶进行液体复苏的RCT共38项。结果显示,与使用其他复苏液相比,使用HES并不能降低患者死亡率(RR=);在排除了博尔特(Boldt)进行的7项研究(存在科学性违规行为)后,HES显著增加患者死亡(RR=)和发生急性肾功能衰竭风险(RR=),并增加肾脏替代治疗的使用(RR=)。论文2月20日发表于《美国医学会杂志》[JAMA 2013,309(7):678]。同期评述用HES 行液体复苏:弊大于利意大利圣心天主教大学 安东内利(Antonelli)和山德罗尼(Sandroni)本次发表于JAMA的荟萃分析提出了一些值得讨论的问题。⑴ 本结果与2012年发表于《科克伦系统综述数据库》的研究结果并不一致。后者未显示HES引起死亡率增加,且排除或不排除Boldt 的研究对结果无影响。当然,该分析仅比较HES 与晶体液,且仅纳入2项Boldt 的研究(死亡率为零);而本研究纳入7项Boldt的研究(与上述2项不同),且对照组报告了84例死亡,故会对整体结果产生影响。此外,本研究纳入了3项2012年发表的研究,其中的6S 研究为首个报告使用HES显著增加死亡率的研究,且样本量大。⑵ 人们试图通过降低HES 分子量和羟乙基化程度来减少其肾毒性,然而,与林格乳酸盐相比,仍更易造成肾小管损伤。本研究的亚组分析亦证实了这一点。⑶ 本研究强调了在患者安全性评估中出现科学性违规的严重性。2011年3月,14位医学期刊主编发表联合声明,公布了Boldt事件及德国莱茵兰法尔茨国家医学协会(LAK-RLP)对Boldt 医生进行的临床试验的伦理审查结果。其中,80项研究被认为不符合伦理标准或存在科学违规行为。本研究验证了HES的肾毒性,且为首个证明用HES进行急性液体复苏增加死亡率的荟萃分析。 但因比较的液体种类繁多,未能就胶体与晶体液间长期存在的争议提供直接证据。未来须行优质RCT来评估HES 在《AnesthesiaandAnalgesia》上的论文因为数据太完美而露出了马脚,不断有读者写信进行质疑,岳凯姆?伯特置之不理的态度使得杂志主编最终对此展开了调查。2010年10月,调查结果让人大吃一惊:此项所谓的临床研究并无伦理委员会同意,甚至连临床试验所需的药品供应商都已经多年未供货。随后的调查则一步步证明了岳凯姆?伯特90多篇论文均为编造的产物。因此,羟乙基淀粉的安全性和有效性被重新考虑。全球顶级的3大临床医学期刊做出了对羟乙基淀粉的不利表态:在有效性方面,羟乙基淀粉不高于同类产品,却会显著增加肾损伤的危险。有效性不高,副作用更多,价格是同类产品10倍,性价比低,导致欧洲和美国危重学会也相继作出了不推荐甚至反对使用的决定。国家食药监总局:目前未关注/据《每日经济新闻》记者了解,在国内,羟乙基淀粉是重症科、麻醉科等科室的常用药物。“欧洲是羟乙基淀粉的发源地,之前使用很多,但是丑闻爆发后用得很少,而美国一直就使用得很少。”中华医学会重症学会的有关人士对《每日经济新闻》记者表示,重症学会不少人对此已经达成共识:临床上谨慎使用,少用。然而,让上述人士担忧的是,很多中国的医生,包括麻醉、外科医生,还不知道羟乙基淀粉的不良作用以及最新证据。在国内最大的面向医生的专业网站丁香园上,揭露岳凯姆?伯特作假以及欧美各国强烈反对一事,呼吁医生们谨慎使用的帖子在麻醉科版面发布后,引起诸多争论,有医生表示“应将此药直接扔进垃圾桶。”也有许多麻醉科医生表示困惑,称此药是常用药,在临床麻醉第一天就接受诸多优点的灌输,还有医生称前几天还受到培训称此药多么好。昨日,记者以麻醉师的身份向国家食药监总局进行咨询,其表示,他们并没有对此药进行关注。记者试图以普通民众的身份了解羟乙基淀粉不良反应信息时,被相关监管部门告知,不良反应信息的内容只是作为对药品进行评价的参考,不对公众开放。不过,国家食药监总局的官方网站显示,2005年的药品不良反应信息通报里,当时的国家药监局就曾发过关于羟乙基淀粉40、20氯化钠注射液对肾功能损害的通报,1988年至2005年3月,国家药品不良反应监测中心病例报告数据库中,有关“706代血浆”、羟乙基淀粉40氯化钠、羟乙基淀粉20氯化钠注射液的病例报告共84例,其中肾功能衰竭1例。当时的国家药监局结论称,建议羟乙基淀粉40、20氯化钠注射液可导致肾损伤,医务人员应严格掌握适应症,避免长期大量、大剂量使用。业内:产品入华或涉嫌商业贿赂/中华医学会重症学会的人士表示,丑闻爆发并未对我国销售造成很大的影响。据了解,2004年,羟乙基淀粉的销售就达到4亿元人民币,现在最保守估计也有15亿元。《每日经济新闻》记者查询发现,在2004年费森尤斯卡比国际临床输血替代学术进展论坛上,岳凯姆?伯特作为专家现身,阐述贺斯 (羟乙基淀粉)的好处。贺斯是费森尤斯卡比公司生产的羟乙基淀粉的商品名,这家公司是目前国内和国际上代血浆市场中份额最大的公司。业内人士表示,国际上不推荐使用,国内却反而用得多,商业贿赂可能起到了作用。一篇网帖的发贴人自称深圳市某医院麻醉科医生,指出该科室一位任职7年的副主任收受高额回扣敛财,涉及金额数千万元。在其所列的收受贿赂的名单中,费森尤斯卡比赫然在列。此事经媒体曝光后,涉事医院很快展开了调查,深圳市纪委也在此前收到举报信,正准备进行调查,并称其违规的事情属实,不过具体金额仍待求证。对于是否进行告知问题,贿赂是否属实的问题,费森尤斯卡比公司未予回应。科伦药业:将密切追踪临床反应在血液及其制品日益紧缺的情况下,国内还有不少厂商对羟乙基淀粉存在浓厚的兴趣,其中包括国内输液巨头华润双鹤和科伦药业。在华润双鹤2009年半年报中,羟乙基淀粉(盈源)被列为重点产品。联讯证券在发表的研报中称,羟乙基淀粉也被列为华润双鹤非基础性输液的重点品种。研报透露,2011年,治疗性输液占公司大输液收入的比例为20%,销量从2010年2500万瓶迅速增长至2011年5200万瓶,同比增长105%。科伦药业对羟乙基淀粉也投入了非常大的精力,且销售额已经相当靠前。在2012年的年报中,科伦药业称其另一产品——羟乙基淀粉乳酸钠林格注射液产业化,已获得发改委通用名化学药发展立项专项。对于是否知道此产品存在副作用,以及是否对产品进行过相关的临床试验,验证其安全性和有效性,截至记者发稿,华润双鹤未给予回复。而科伦药业给《每日经济新闻》记者回复显示,对于岳凯姆?伯特造假和国外学术界对于羟乙基淀粉的反对态度,科伦药业知道得一清二楚。此外,科伦药业还承认羟乙基淀粉在国外的研究中确实出现很多问题,特别是针对肾功能损伤这一副作用。对此,公司将密切追踪该药品的临床反应。不过,科伦药业认为国外权威杂志及危重学会的论断矫枉过正,此产品也未被勒令召回。同时,解放军总医院重症医学教授林洪远也对国外结论提出了质疑。 国内瑞金医院认为此产品还有一定的优势,且国内不少临床医生仍支持配伍使用,所以对产品仍持积极态度。羟乙基淀粉在急救中的临床应用山东省平度市人民医院急诊科、山东省平度市人民医院骨科共同发表论文,旨在探讨羟乙基淀粉130/氯化钠注射液复苏方法对早期失血性休克的临床疗效。研究指出,采用羟乙基淀粉130/氯化钠注射液复苏方法治疗早期失血性休克可增加有效循环血量,改善组织器官灌注,降低病死率。该文章发表在2012年第35卷第24期《中国医师进修杂志》上。 将52例早期失血性休克患者按随机数字表法分为复方氯化钠注射液组(对照组)和羟乙基淀粉130/氯化钠注射液组(治疗组),每组26例,观察比较两组患者治疗效果,治疗前和治疗后60 min的平均动脉压、呼吸频率和心率以及血钠、血氯和凝血酶原时间。结果显示,治疗组和对照组治愈率分别为(22/26)和(10/26),病死率分别为(4/26)和(16/26),两组比较差异均有统计学意义(P<)。治疗组治疗后60 min平均动脉压、心率和呼吸频率均较对照组明显改善[(±)mmHg(1mmHg =)比(±)mmHg、(±)次/min比(±)次/min、(±)次/min比(±)次/min,)。

Joseph Heller’s Catch-22 is one of the most acclaimed novels of the twentieth century. It is a black comedy about life in the military during World War II. It features bombardier John Yossarian, who is trying to survive the military’s inexhaustible supply of bureaucracy and who is frantically trying to do anything to avoid killing and being killed. Heller was able to use his own experiences in the Air Force during World War II to create this character and the novel.

Even though Catch-22 eventually became known as a great novel, it was not originally considered one. When it was first published in 1961, the reviews were tepid and the sales were lackluster. It was not well received at this point at least in part because it presented such a cowardly protagonist at a time when World War II veterans were being lauded for their selfless courage.

Within a few years of the release of the book, as an unpopular war in Southeast Asia was heating up, Heller’s Catch-22 found a new audience eager to enjoy the exploits of Heller’s war-averse protagonist. It was within the framework of this era that Catch-22 was newly discovered, newly examined, and newly credited as one of the century’s best novels.

Let me talk a bit about the expression catch-22. Do you understand what a catch-22 is? This expression is so well known now that it has entered the American lexicon: well, a catch-22 is a situation that is unresolvable, one where there is no good choice, not best path to take.

In Heller’s novel, the catch-22 is a very specific catch in a very specific situation. The situation in which the protagonist found himself was that he wanted to get out of combat by declaring himself insane. So you see that in this situation, there was a very specific catch. In American culture now, though, this expression is used more generally. It refers to any situation where there’s a catch, where there’s no solution, where there’s no way out.

One more bit of information about the expression catch-22, about the number 22 in the expression. This number doesn’t have any real meaning; it just signifies one in a long line of catches. Heller really could have used any number; it didn’t have to be 22. When Heller was first writing the book, he used the number 14; the book was originally titled Catch-14. Then, in the production process, the number was changed to 18, so the title was Catch-18. But then there was a problem with the number 18 because there was another book with 18 in the title, so Heller’s title became Catch-22.

Hemophilia is a condition in which the blood either clots slowly or fails to clot at all. Most people who get a little cut on a finger can put a bandage on the cut, and the cut on the finger will heal because the blood will clot. A blood clot forms from the polymerization of protein fibers that circulate in the blood. A number of protein factors take part in the process, and it is necessary for all of the protein factors to function correctly for blood to clot. Hemophilia exists when any of the factors is either missing or not functioning.

The most common kinds of hemophilia are hemophilia A (or classic hemophilia) and hemophilia B (or Christmas hemophilia), which was named after the first person known to have contracted it. Hemophilia A occurs when clotting factor 8 is not functioning properly; 85% of those who suffer from hemophilia have hemophilia type A. Hemophilia B occurs when factor 9 is not functioning properly; almost all of the rest of those who suffer from hemophilia have hemophilia B.

Hemophilia is generally passed from mother to son, though sometimes it seems to develop spontaneously in some women. Women carry recessive gene but do not generally develop the disease. A mother who carries the defective gene may or may not pass it on to her children. Probably the most famous case of hemophilia is often called Royal hemophilia. Researchers analyzed urine samples from 2,350 women who participated in the long-runningNational Health and Nutrition Examination Survey, a nationally representative sampleof Americanwomen. They were looking for concentrations of chemicals known as phthalates,which are oftenfound in personal care products and in adhesives, electronics, products used tomanufacture cars, toys, packaging and even some coatings for medications.

Phthalates are considered "endocrine-disrupting" because they can alter normal regulationofcertain mechanisms in the body, including hormone regulation, and have been tied inpreviousresearch to diabetes and obesity risk, Dr. Kenneth Spaeth, director of theOccupational andEnvironmental Medicine Center's department of population health at NorthShore UniversityHospital in Manhasset, ., told HealthPop. He was not involved in the study.

The researchers found that women with the highest concentrations of two types of phthalates -mono-benzyl phthalate and mono-isobutyl phthalate - were nearly two times more likely tohavediabetes compared to women with the least amounts of these chemicals. Women withmoderatelyhigh levels of the phthalates mono-n-butyl phthalate and di-2-ethylhexyl phthalatewere 70 percent more likely to have diabetes compared to their counterparts.

The findings were published in Environmental Health Perspectives, a journal published bythegovernment's National Institute of Environmental Health Sciences.

"This is an important first step in exploring the connection between phthalates and diabetes,"saidDr. Tamara James-Todd, a researcher in women's health at Brigham and Women's HospitalinBoston, said in a press release. The researchers could not prove that phthalates causeddiabetes orhaving diabetes increased concentrations of the chemicals in a person's body.

"We know that in addition to being present in personal care products, phthalates also existincertain types of medical devices and medication that is used to treat diabetes and this couldalsoexplain the higher level of phthalates in diabetic women," she added. "So overall, moreresearch isneeded."

One of the problems is that chemicals like phthalates are practically unavoidable, accordingtoSpaeth.

"These chemicals are unfortunately ubiquitous," Spaeth explained. "It's pretty clear fromstudiesthat we're exposed all day long to these various household or personal care products."

Spaeth says it's a real challenge to reduce phthalate exposure because sometimes thechemical is ametabolic byproduct of another ingredient or a product label may not say itcontains phthalatesonly for phthalates to be found in the packaging the product came in, whichdoes not need to bementioned on a product label.

"It's really hard to make informed decisions about these kinds of things," he said. "Maybe we'llgetto a point when the health effects are more widely recognized, that there will be incentivetochange how products are made and packaged."

However Spaeth did say research has shown phthalates can find their way into householddustand people sometimes ingest them that way, so simple steps like frequent vaccuming anddusting, or washing your hands regularly before eating may decrease risk.
